A nurse will be assigned to you to help you understand your condition and your physician’s treatment recommendations. It also provides you with no cost tools and educational materials to help make living with your condition easier.

AccordantCare also helps you comply with proven methods of care, improving your quality of life.

Employees and their eligible dependents that are provided prescription benefits through the Caremark Prescription Plan, and have one of these chronic medical conditions are invited to participate in the AccordantCare program. AccordantCare is voluntary, completely confidential and provided at no cost to you.
